LCSW vs psychologist, comparing two common therapy licenses

Licensed clinical social workers and psychologists both provide therapy, both are licensed by the state and both are common choices for outpatient care. Their training differs in length and emphasis, and that shapes some of what each one does. This guide compares the two credentials in general terms to help you read listings and ask informed questions.

These are broad descriptions. Each state sets its own requirements, and titles vary. Some states use LICSW or LCSW-C, for example, for the clinical social work license.

What an LCSW is trained to do

Clinical social workers start with a Master of Social Work. Clinical coursework covers assessment, psychotherapy and mental health, and the profession as a whole has a strong tradition of looking at a person's situation in context: family, work, community, culture and access to services. After graduation, a social worker completes supervised clinical practice and passes a clinical exam before the state grants the clinical license.

Clinical social workers work in private practice, hospitals, schools, community mental health centers, substance use programs, hospice and many other settings. Many provide ongoing individual, couples, family or group therapy. In many states LCSWs can assess and diagnose mental health conditions within the scope of their license.

What a psychologist is trained to do

Psychologists complete a doctoral program in psychology. A PhD program usually puts more emphasis on research, and a PsyD program usually puts more emphasis on clinical practice. Both include extensive training in psychotherapy, psychological theory and psychological assessment, followed by a supervised internship and, in most states, further supervised experience before licensing.

Psychological testing is where the two credentials differ most clearly. Psychologists are trained to choose, administer and interpret standardized tests, for example evaluations related to learning, attention, memory or personality, and to write formal reports. People are often referred to a psychologist when a school, employer or court asks for such a report.

Many psychologists in private practice spend most of their time on therapy. You can browse them on our psychologist near me page.

How the work can look day to day

For someone seeking regular talk therapy, sessions with an LCSW and with a psychologist can look very similar. Both may use well known approaches such as cognitive behavioural therapy near me, dbt therapy near me or emdr therapy near me, depending on their additional training. Both often hold sessions of around 45 to 60 minutes, in person or online.

The differences tend to show up at the edges.

  • An LCSW may bring particular experience connecting clients with community resources and services.
  • A psychologist may offer formal assessment alongside or instead of therapy.
  • Each may specialize in specific groups or concerns, which matters more for fit than the license alone.

Cost and insurance in general

Fees vary widely by city, setting and provider. Psychologists often charge somewhat more per session than LCSWs, reflecting their longer training, but this is a tendency and not a rule. Formal psychological testing is usually priced separately and can cost considerably more than a therapy session, because it involves several appointments plus scoring and a written report.

Both LCSWs and psychologists can be in-network with insurance plans, and many plans cover both. Some providers are out-of-network and give clients a superbill. If you pay yourself, you have the right to a good faith estimate under the federal No Surprises Act. Which one might suit you

This is a personal choice, and a provider can help you think it through. Some general points many people consider:

  • If you are looking for ongoing therapy, both credentials are qualified to provide it, and fit, experience and availability may matter more than the license.
  • If someone has asked for a formal psychological evaluation, that is typically the work of a psychologist.
  • If cost is a concern, LCSWs may sometimes have lower fees or more in-network availability in your area, though this varies.
  • If you have questions about medication, a medical provider such as a psychiatrist is the right person to ask. Our guide on psychiatrist vs psychologist explains why.

Questions to ask either provider

  • What is your license, and in which states are you licensed?
  • What concerns and age groups do you work with most?
  • Which approaches do you use?
  • Do you offer assessment or testing, or only therapy?
  • What is your fee, and do you take my insurance?
  • Are your sessions online, in person, or both?

Telling them apart on NearbyTherapy

Every listing on NearbyTherapy shows the credential recorded in the public federal NPPES NPI Registry, updated monthly, so you can see at a glance whether a provider is a clinical social worker or a psychologist. Claimed profiles carry a License verified badge and add details the provider entered, such as concerns, approaches, insurance and fees. Searching is free for patients and no account is needed. Associate licenses and supervision

Both professions have an early career stage that you may see on a listing. A social worker who has finished the MSW but not yet completed supervised clinical hours often practices under a title such as LMSW, LSW or an associate clinical social worker license, depending on the state, and sees clients under a licensed supervisor. A psychologist who has finished the doctorate but not yet been fully licensed may work as a postdoctoral fellow or under a supervised or provisional title.

Seeing a clinician at this stage is common and can be a way to access care at a lower fee, especially through group practices and training clinics. It is reasonable to ask who the supervisor is, how supervision works and whether the supervisor is involved in planning your care. The provider should be able to explain this clearly.

Is an LCSW a real therapist

Yes. An LCSW is licensed by the state to provide psychotherapy. Clinical social workers provide a large share of outpatient mental health care in the United States.

Can an LCSW diagnose

In many states LCSWs can assess and diagnose mental health conditions within the scope of their license. The exact scope is set by state law.

Can an LCSW do psychological testing

Formal standardized psychological testing is generally done by psychologists. LCSWs may use brief screening tools as part of an assessment.
